Service request forms
These service request forms are templates which clients are free to use when submitting work to ASAMS. They provide prompts for most information required.
DR1 – Acceptance criteria taken directly from referenced specification. If more than one specification is referenced then the acceptance criteria will be the most stringent of all referenced specifications. An un-constrained simple acceptance criteria has been applied using the zero guard band method. The major sources of uncertainty have been accounted for in the standardised test procedure. Uncertainty budgets available upon request.
DR2 – Acceptance criteria advised by client. Uncertainty and risk of the result has been taken into account and details of this are provided with the report. Details of the guard band used are also supplied with the report.
DR3 – ASAMS derived acceptance criteria. Uncertainty, guard banding and risk considerations documented and supplied with report.
DR4 – Other decision rule applied. Full details supplied with report.
